» Helping you to access health care in a supported, efficient and effective manner
Caring for a family member is a life-changing commitment. Carers make a vital contribution, often providing most of the support the person they care for needs.
Access to Health Care is vital to enable Carers to maintain their own health to ensure they can continue in their caring role.
The aim of the Access to Health service is:
- To negotiate with Health professionals to enable appointments and treatments to be arranged at times and venues that can fit in with your commitments.
- To organise, through staff and volunteers, support for the person cared for – giving you peace of mind whilst you attend appointments/receive treatment.
- To accompany you and the person you care for to health appointments.
- To provide a limited “Sitting Service” whilst you attend a medical appointment.
- To assist you to attend either for day hospital or inpatient treatment by liaising with appropriate agencies.
- To offer signposting to other appropriate agencies.
We Can:
- Provide support
- Help co-ordinate multiple appointments
- Assist with arranging transport
- Ensure appointments are community based when possible
